Overview
Lowering of LDL-cholesterol (LDL-C) levels remains an effective strategy for preventing atherosclerotic cardiovascular (CV) events in adults aged ≥ 75 years. In clinical practice, however, lipid-lowering treatment in older adults is complicated by comorbidities, polypharmacy and a higher prevalence of statin intolerance. Bempedoic acid (BA) is an oral ATP-citrate lyase inhibitor with proven LDL-C lowering and CV benefit in statin-intolerant patients. New age-specific analyses show that the LDL-C-lowering effect of BA is preserved in patients aged ≥ 75 years, i. e. placebo-corrected LDL-C decreased by 22.6 %, with no evidence of heterogeneity in the MACE-4 (compared to younger subjects). These findings support BA as a treatment option in appropriately selected older patients.
